TEMPLATE NEEDED! 81/2" x 2"

WATER BOTTLE... with the "Nutrition Facts" already listed... but that I can modify with our wedding information. I have the labels (5 per sheet) but cannot find the bloody template!

HELP!

I believe I have this at home in my notebook. I'll look and post if someone else hasn't done so by then!
