Simple Ceremony Template

I am looking for a template to make my program for the ceremony.

 

I did a search, and did not find the actual wording, just templates to make the fans or tags.

 

Anybody have a word file of what they used on their programs? Something simple- we are doing a symbollic ceremony with a small wedding party.

What I am looking for is a single sheet that will fit on a raffia fan. I know I saw some on here, but can not find them!
