Custom Applique

Does anyone know of someone that can make a custom rhinestone applique. I have spent hours searching the internet but I can't find what I want or something even similar to the shape. I appreciate the help! I included a picture I found on the internet of what I am wanting the applique to look like (or as close as possible). I already have a dress, just looking to have this same/similar embellishment.
