Who to cut-off from guest list?

My wedding is super far away, but I'm curious, what are your guidelines for including or not including people in your guest list? Should you invite people that invited you to their wedding even if you don't see them that often anymore? What is the proper way of going about this?

We invited immediate family (parents and siblings), aunts and uncles (only parents siblings) and first cousins. Only 3 of my first cousins have children (second cousins) for a total 4. We have limited ourselves to 5 friends each...spouse included. Everyone else was cut off. We just tell people it's an intimate affair.
