"Mint to Be" Mint Matchbook Help

Hi Ladies! I was hoping one of you creative brides would be willing to help me. I am super un-creative. I was on pinterest and saw these super cute mint matchbooks. They look pretty easy to put together if only I had a template. I've searched the web for a template but cannot find one. The link that I'll provide below has a template on it but I cannot download it. When I do, it prints out HUGE! I was also hoping to find a template where i can put our wedding date and "Mint to Be" somewhere on the matchbook. Does anyone have a template they are willing to share or is anyone creative enough to help me make one? I think this is a cute and cheap idea for the reception or for welcome bags.. It looks so simple, but yet, I'm having trouble with the template ;( Here's the link I'm interested in.
