I didn't get one either.  I emailed her asking how I would go about adding a DJ and I haven't heard anything back yet.  It's only been a few days, so I'm not too worried.

In case anyone is wondering, here is the DJ form that Pilar sent me. I'm using whichever DJ they provide so I am not sure if it makes a difference but here's what I have.

This is the same form I received.

 

It probably isn't a huge deal if you didn't get the form...just make sure that you have the songs you want either on a cd or an ipod. The DJ should have most of the songs you want unless they aren't well known so it's good to have them with you.

I love the cake in the last photograph.  I am getting married in March and the cake is perfect for my blue colors.  I'll have to remember to print out a copy so I can show Pilar.
