Thanks for all the compliments everyone!!  Once I started, it just became so easy and I was finished before I knew it!

 

I didn't use a particular template in PowerPoint.  All I did was use a blank slide and inserted a table with 3 columns and 1 row.  I expanded to fit the whole slide and there you go!
