I ordered mine from Etsy seller bbond0520, she was great to work with.  Those of you more creative DIY'ers could probably duplicate these pretty easily though!  Here they are:

I've found a lot of variation between what each store stocks.  Yes, my Party City had the white ones and I also recently saw them at Kroger (a large grocer in the southeast).  Here is what its called - maybe you can do a search on-line to see who has them.

 

Right now I'm trying to find plain white matchbooks and no one carries those
