Basically, in order for an American to go to Cuba, they would have to cross the border into Canada or Mexico, either by driving or by flying, then take a completely separate flight into Cuba. There is no way for customs to track that.
What a pain that must be! I'll be happy for all of you American folks if they ever do loosen up these restrictions for you, that's so crazy that you'd have to go through all of this just to take a trip to Cuba! Until then, I guess it's just more room at the beach for the rest of us!
