Hi Shanelle! I did a civil ceremony. I went through We Do and I we sent them: copies of our birth certificates, copies of our passports, and two notarized "Single Status Affadavits". They send you the wording and you print it, sign it, and have it notarized. I'm pretty sure that's all I sent them (plus a check!). They had it back to me, translated, in about a week and a half. Then when you go to the Dominican you bring all the documents from them, as well as your original birth certificate and passport. I'm pretty sure that's all, but they could tell you better.
