NEED HELP! confused with paper work needed!!

All BEACHES/SANDALS brides out there!!! Im so confused with the paper work we need to have done!

I know we each need to fill out an affidavit saying we both have not been married and we are US citizens.. and I can go to any bank to have that notarized correct? What other paperwork do they want and need besides a photo copy of our passports... which we still need to get!

 

HELP! so confused! I know Im prob freaking out over nothing but the paperwork is due in october and it is quickly approaching!!!!

Original state-issued birth certificate AND a valid passport or driver's license ...

do they literally want my original birth certificate?!

 

All pages of the court certified final divorce decree... Im not divorced...

 

State issued death certificates with related marriage certificate.. Im not dead..

 

Legal name changes or legal adoptions papers if applicable..

Do I need to change my name before we go? They need this paperwork in October, so do I need to change my name before then?
