there's a good thread that mikkistreak (maria) posted with detailed instructions on how to do it. you can find it by using the advanced search and searching her username.

Check out the post by JanaBrook (sp) around Oct 12th 2007. She linked the powerpoint monogram version. 
Check out some of the earlier threads. They are very helpful.
